Care Management
Comprehensive Assessments:
Our trained Care Managers conduct a thorough assessment of the individual’s physical, emotional, and social needs. Based on the assessment, we develop a tailored care plan to address immediate and long-term needs.
Care Coordination:
We can schedule medical appointments and accompany the client as needed, ensuring clear communication with healthcare providers. We also help manage and organize medications to facilitate proper adherence to prescribed treatments. Our services also include coordinating other services such as in-home care, physical therapy, and other necessary support.
Emotional and Practical Support:
Family caregivers carry a heavy load of responsibilities. We can provide relief by taking on some of the caregiving responsibilities, allowing the caregiver to take breaks and reduce stress. We can also offer guidance and practical advice on managing day-to-day caregiving tasks and making informed decisions.
Home Safety Assessments:
We assess the home environment to identify potential hazards, suggest modifications and adaptations to improve safety and accessibility and assist in making any needed changes.
Health and Wellness Monitoring:
We conduct regular visits and check-ins to monitor the well-being of the older adult, keeping track of health status changes and ensuring compliance with medical recommendations.
Advocacy:
Our goal is to advocate for older adults, seeing that their preferences and rights are respected in all care decisions. We can serve as a liaison between the family, healthcare providers, legal advisors, and other professionals whenever necessary.
Crisis Intervention:
Our Care Managers can provide support and assistance during emergencies such as hospitalization or sudden illness. We also help with crisis planning and develop and implement emergency plans to ensure safety and swift action when needed.
Family Support and Education:
One of our vital roles is to offer emotional support and encouragement to family members dealing with the stress and responsibilities of caregiving. We strive to educate families about the aging process, medical conditions, and effective caregiving strategies.
Transition Assistance:
When the time comes for a change, we assist with transitions between care settings, such as from a hospital to a home or an assisted living facility. Managing the logistics of moves and ensuring continuity of care during transitions can ease the emotional toll on the client and their family.
Resource Connection:
Our years of experience in senior healthcare enable us to connect families with community resources such as support groups, financial assistance programs, and other services. We also provide information about available options and help families navigate complex systems.
Long-Term Planning:
Our team helps families plan for the future, including long-term care options, financial planning, and legal considerations. We desire that families have peace of mind by having a clear and actionable plan for their loved one’s care.
Documentation and Communication:
We maintain detailed records of the older adult’s medical history, care plans, and services received and keep family members regularly informed.
Fiduciary Services
In addition to being an Aging Life Care manager, Cindie Van Noy is also a licensed professional fiduciary. Caring for seniors is her life’s work and passion, and combined with her many years of experience, she knows how to navigate and deal with healthcare providers, caregivers, government agencies, insurance companies, banks, etc., and most importantly, with clients and their families.
Cindie will serve as agent under a durable power of attorney for finance and/or under an advance health care directive, trustee, successor trustee, conservator, administrator of will and executor of estate.
